MAIL IN REPAIR INSTRUCTIONS:
1.Request Return Authorization Number by calling 800-551-3676, 856-767-6627 or email your request with the following information.
Full Name, Address,City,State,Zip. (No PO Box)
Day Phone No., email address
Make and Model of instrument to be repaired.
Serial number required if instrument is under Factory Warranty.
2. You will recieve a RA Number which must be displayed on your shipping label.
3. Include detail description of problem.
4. Rates are $60.00 per hour, max of 2 hrs. otherwise an estimate is foward to above address.
5.Include 1st hour payment of $60.00 with your order.(Check,MC,Visa.accepted.)
6. Repaired item will be returned by UPS Ground unless otherwise specifed. Shipping charges not included.
